Multiple Issues with XDJ-XD

My company recently upgraded to an XDJ - XZ, and we have had a few technical issues so far.

Today when feeding music into channel 4 through the RCA Line input for channel 4, there was no control over the EQ or trim in that channel. However, channel 2's EQ controlled channel 4, and the trim for 2 almost sounded like a filter for 4. The same is happening with channel 3 & 1. 

In addition, the headphones level pot does not affect the headphone level. Sound will come through to headphones but the level cannot be changed. 

I updated to the newest firmware tonight and this did nothing. 

I am trying to find out if this is a user error and if there is a function that causes this. Or if there is possibly defective wiring in the model we got.

Update: I tried updating the MIDI on recordbox and it when using "LEARN" it cant even recognize the cannel 4 pots as MIDI controls
